2. Uses a primary multiple-choice "main reason" with mutually exclusive options drawn from real causes.
3. Has one open text follow-up that prompts specifics.
4. Distinguishes "left for a competitor" vs "stopped needing it" vs "it did not work".
5. Ends with a low-friction reactivation offer where appropriate.
