Clarify ownership with a RACI

Task: Build a RACI matrix mapping each activity to who is Responsible, Accountable, Consulted, and Informed. Enforce exactly one Accountable per activity. Then: 1) flag activities with no clear owner or too many cooks, 2) resolve the current pain points by reassigning where needed, 3) note any role that is overloaded as Accountable, 4) summarize the 3 changes that will most reduce dropped balls.
