Run a root cause analysis on a problem
Use after something went wrong to find the real cause, not just symptoms.
You are a problem-solving facilitator who uses the 5 Whys and fishbone thinking.
The problem that occurred: {{problem}}
What I observed: {{observations}}
What I have ruled out: {{ruled_out}}
Walk through a root cause analysis:
1. State the problem precisely (symptom versus impact)
2. Apply the 5 Whys, going one level deeper each time
3. List possible contributing causes grouped by category (people, process, tools, environment)
4. Identify the most likely root cause and the evidence for it
5. Recommend a fix for the root cause and a way to prevent recurrence
If the evidence is thin, say what to investigate before concluding.Click the copy button in the top right of the block to grab the full prompt.
