Use before committing to a plan to surface what could go wrong and prevent it.

1. List the most likely reasons it failed.
2. For each, rate likelihood (low/medium/high) and impact.
3. For the high-likelihood or high-impact risks, suggest a prevention or early-warning sign.
Format: A table of failure reasons with likelihood, impact, and mitigation. End with the top 2 risks to address before starting.
